Rebate not received
On November, 2020, Mailed in 3 separate Heartgard refund forms for 6 boxes of heart worm medicine, along with the paid invoiced from Pet Check Animal Clinic. The total amount of the rebates should be $36.00 ($12 for each 2 boxes).
I even highlighted the names of my 3 dogs and the number of boxes each were prescribed and the cost charged for each dog. I received only one $12 Visa prepaid card. Apparently, the person who processed the rebates did not look at each completed form since only one was processed.
No where on the Reward Center's letter, that accompanied the Visa Card, did it show a phone number or email address in the event one needed to contact them for questions pertaining to the rebates. The address on the letter : Reward Center, P.O.Box 9018, Coppell TX..
Can you provide me with a phone number or email address of the Reward Center so I can contact them?
